Check Request Form
Deacons and other leaders - Use this form to submit receipts and be reimbursed for ministry expenses.
Facility Request Form
OCF attenders only - use this form to request permission to use any classrooms or OCF facilities for special events. Office staff will check for availability of rooms and for any calendar conflicts.
Please note the instructions on the forms. The Facility Request can be faxed back to the office (310-414-4577). The Reimbursement form CANNOT be faxed back if you need to attach receipts - you must mail it in with your receipts, or bring it to church during office hours or Sunday or Wednesday service times. Thank You.
